Visit MyHealthSpace to fill out your consent forms before you arrive. You can also find and fill out other forms including the Health History, Treatment Agreement & Notice of Privacy Practices and Permission to File Insurance.
Services for Students
The Allergy, Immunization and Travel (AIT) clinic provides a range of essential services for students. The AIT clinic is dedicated to delivering courteous service and prioritizing the health and safety of its patients.
Cost for Services:
We charge a fee for these services, our office will courtesy file insurance, but we are only in network with the Student Health Insurance Plan. For travel vaccines, fees will depend upon consultation and the number of vaccines needed.

The lab provides walk-in health screenings and rapid in-house testing. We also partner with outside reference labs to provide a full range of testing. Call 803-777-3175.
Request a Test
You can request any of the following screenings yourself through MyHealthSpace or at any kiosk in the Center for Health and Well-Being. Your doctor will need to order any test that's not on this list.

Outside Lab Orders
Licensed providers may fax orders directly to the laboratory at 803-576-5668 or 866-245-2537. There is a $25 handling fee for all outside orders. All orders must include:
- Two patient identifiers (name, date of birth or social security number)
- The health care provider's name and signature
- The health care provider's phone and fax numbers
- Diagnosis code (ICD10)
- Clearly listed tests (LabCorp test numbers may help clarify panel requests)

Care Options
- Questions and Referrals - Call 803-777-0564 to talk to the care team nurse about your health care or your treatment options, including PrEP and referrals to community experts for gender-affirming surgery or hormonal therapy.
- Medical Providers - If you are more comfortable working with a particular health care provider or a provider of a particular gender, let us know when you schedule your appointment.
- Confidentiality - All student visits are confidential. Mental and physical health records are separate from other university records. We will not release your protected health information without written permission except as required by law or to protect you or others from a serious threat to health and safety.

Our primary care team offers a convenient and affordable on-campus option for general medical issues. Primary care can treat common illnesses and injuries and provide comprehensive care for chronic medical health concerns. Call 803-777-1546.

Services:
Radiology is staffed by radiologic technologists who perform diagnostic X-rays and EKGs as ordered by health care providers. Outside orders are accepted, also. To chedule an appointment, call 803-576-6383 or visit MyHealthSpace.
- Referrals

- If you have an order from an outside provider, it must include your full name, date of birth, the exam being ordered, a diagnosis code, and the health care provider’s name, credentials, phone number, and fax number. We will ask that you bring this order with you when you present for services.
- Copies of X-rays

- Patients may request copies of their X-rays at no additional cost. Images are copied to a CD that can be delivered directly to the patient or mailed at the patient’s request.
- Cost and Insurance

- X-rays and EKGs are not covered by the student health fee. Cost varies.

If you have a sport-related or activity-related injury, our sports medicine physician will help diagnose and treat conditions that affect your daily activities and help you reach your performance and recovery goals.

Temporary Conditions and Injuries
- Students experiencing a temporary condition or injury may qualify for assistance if the condition or injury negatively impacts the ability to succeed in the classroom. Our team works together with the Student Disability Resource Center to provide recommendations to support student’s ongoing academic success.
Sports Medicine Fellows
- We are proud to be a part of the diverse training of the Prisma Health/USC School of Medicine Sports Medicine Fellowship.
Students have access to a full range of women's health care services, including annual visits, rapid in-house laboratory testing and screening, contraceptives and pregnancy counseling.
